Necessary Paperwork Should Your Client Choose BCAM
All agents must complete the necessary paperwork including an Acord Application for their potential new client. A complete list of all BCAM forms can be found at:
On line 1 of Application Checklist, verify that the prospect is a member of one of our sponsoring associations Home Builders Associations of Mississippi (HBAM) or Mississippi Associated Builders and Contractors (MSABC). Indicate on our BCAM checklist form which association they are a member of. If they are not a member of one of these associations, they must join one in order to purchase our workers' compensation insurance. We will, however, admit a new member if they are in the process of joining one of these associations.
Next, complete items 2-4 on the checklist.
Upon completion, please send all of these documents to BCAM's service Company, Amfed Companies, LLC by fax at 601-427-1535 to the attention of BCAM's Underwriting Department or send it in an email to misi.moreland@amfed.com. You must also send a check for the premium and/or escrow deposit to Amfed Companies, LLC, P.O. Box 1380, Ridgeland, MS 39158. Checks should be made payable to BCAM.
